2) The Printable PDF’s below are not digitized yet, but we can hand you a copy if you arrive early, or you may print and fill out.

Patients Who Have Dental Insurance – We welcome dental insurance. We will be happy to answer all of your questions and call your insurance provider to verify benefits. We participate with Delta, United Concordia, and Aetna dental insurance carriers.
